   2.  Full shipping address, including country
   3.  Area code and phone number, including country code if outside USA
   4.  Personal e-mail address (this is the address I will send mailing information to)

After I receive your e-mail with all of the above, an e-mail with a mailing form will be sent to you.  The form will contain instructions, payment address and correct shipping/handling rates (based on which country the package will be shipped to) for your address.
